Dyslexia Screening Tool

A comprehensive screening for parents, teachers, children, teens, and adults

Important: This screening helps identify possible signs of dyslexia by looking at reading-related skills such as sound awareness, decoding, spelling, fluency, and recognition of common high-frequency words. It is not a diagnosis. If results suggest risk, a full evaluation by a qualified professional is recommended.

  • Complete all sections for the most accurate results.
  • For children, a parent or teacher should assist with oral tasks.
  • For adults, complete the form independently or with a support person.
  • Oral tasks (Sections 4–8) should be done in person with the individual being screened.
  • The screening takes approximately 20–30 minutes.

Scoring Guide: 2 = correct independently · 1 = correct with help · 0 = incorrect or unable
