Dyslexia Screening Tool

This screening helps identify possible signs of dyslexia and reading difficulty. It is not a diagnosis. If results suggest risk, a full educational evaluation is recommended.

• Complete all sections for the most accurate results.
• For children, a parent or teacher should assist with oral tasks.
• For adults, complete the form independently or with a support person.
• Oral tasks (Sections 4–8) should be done in person with the individual being screened.
• The screening takes approximately 20–30 minutes.
